Group Claims Process
Claims Procedure:
Claim Submission Options:
The claimant/nominee can email all necessary documents to [email protected]
(Original documents may be requested on a case-by-case basis)
OR
The Claimant/Nominee can contact the Master Policy Holder, who will Collect all documents from the claimant/nominee Intimate the claim to the insurance company on behalf of the claimant
Mandatory Documents Required for Claim Intimation:
Claimant’s statement/Claim form in the prescribed format (Download claim form: https://www.acko.com/life/download/)
Copy of the original Death Certificate issued by the local/municipal authority
KYC documents of the Claimant (Aadhar Card/Pan Card)
Cancelled cheque/Bank statement/Copy of bank passbook
In case of death due to Medical Causes:
Medical records (admission notes, discharge/death summary, test reports, etc.)
Medical certificate signed by the last attending doctor
In case of death due to Unnatural Causes (Accident/Suicide/Murder):
Copy of FIR or Panchanama/Police complaint
Postmortem Report
Viscera Report/FSL report
Final Police Investigation Report/Charge Sheet
Court order
Additional documents may be requested depending on the nature of the claim.
TAT for Claim Settlement:
Death claims will be processed within 15 days from the date of receipt of claim intimation along with all relevant documents.
If investigation is required, settlement will occur within 45 days from the date of receipt of claim intimation

Turnaround Time (TAT) for Claim Settlement:
· Death claims under retail policies will be processed within 15 days from the date of receipt of claim intimation along with all relevant documents.
· If the claim requires investigation, it will be settled within 45 days from the receipt of claim intimation.
Critical Illness, Total and Permanent Disability or Terminal Illness
Claims Procedure:
To register a claim, please submit all necessary documents by emailing us at [email protected]
Mandatory Documents:
1. Claimant’s statement/Claim form in the prescribed form. (Download claim form: https://www.acko.com/life/download/)
2. Attending physician’s statement
3. Attested copies of all medical records
4. Copy of Cancelled cheque/Bank Statement/Copy of Bank Passbook duly attested by the bank
5. Certification of disability along with percentage of disability/ Photograph of the injured reflecting disability, as applicable
TAT for Claim Settlement:
Claims will be processed within 15 days from the date of receipt of claim intimation along with all relevant documents .
If investigation is required, settlement will occur within 45 days of claim intimation.
