Driving Licence Form List

Here are the RTO forms related to the Driving Licence.

  • Form 1A: It is also called a 'Medical Certificate'. This form will include the results of a thorough examination conducted by a  state government-appointed medical practitioner.

  • Form 2: It is an application form for the grant of a Learner's Licence (LL), Driving Licence (DL), renewal of DL, change of address or name in DL and addition of new vehicle class to DL. The applicant has to fill in the type of motor vehicle he or she is applying for. You may also need to fill in other information like the DL history of the candidate, such as ownership of any previous licences, disqualifications, etc.

  • Form 4A: An application form for the issuance of the International Driving Permit (IDP). If you wish to drive a vehicle in other countries, this form will help you obtain an IDP.

  • Form 7: If you wish to apply for a laminate or smart card-type Driving Licence, you need to fill out Form 7 and submit it to the RTO.

Vehicle Registration Form List

Following are the RTO forms related to vehicle registration.

  • Form 20: It is also referred to as the 'Application for Registration of a Motor Vehicle'. This form is useful if you wish to register a motor vehicle. It requires accurate completion of details regarding the motor vehicle, such as the type of body, vehicle type, number of cylinders, seating capacity, and the fuel used in the engine. 

If the vehicle operates without an engine, there is a separate section designated for it. Form 20 also includes the certificate of inspection for the motor vehicle and the office endorsement certificate.

  • Form 26: Also known as the 'Application for the issue of a duplicate certificate of registration', is to be filled out by the applicant in the event of losing the original vehicle registration certificate.

  • Form 27: It is an application for the assignment of a new registration mark to a motor vehicle. It must be filled by the applicant when they need to register the vehicle again, usually due to relocating from one state to another. Some of the documents required along with Form 27 include a No Objection Certificate from the registration authority, a certificate of registration, and a certificate of fitness for the vehicle.

  • Form 28: This form, also known as 'Application and Grant of a No Objection Certificate (NOC)', is used to issue a NOC to the applicant.

  • Form 29: Referred to as the 'Notice of transfer of ownership of a motor vehicle', the form is to be filled when the owner of a motor vehicle intends to transfer ownership to another individual. You can submit this form to the respective Regional Transport Office (RTO) as a notice of the ownership transfer.

  • Form 30: Alongside Form 29, you also need to submit the duly filled Form 30 to the RTO when transferring ownership of a vehicle to another person. Form 30, also known as the 'Report of transfer of ownership of a vehicle', entails filling in details of both the transferor and the transferee.

  • Form 31: This form needs to be filled out by the applicant in the situation where the owner of a motor vehicle has passed away and the applicant wishes to assume ownership of the vehicle. The form necessitates filling in the details of the deceased owner.

  • Form 32: It is an application form for the transfer of ownership if the vehicle is purchased or acquired in a public auction.

  • Form 33: If you wish to change the address in Registration Certificate, you need to fill the Form 33 and submit it to the RTO.

  • Form 34: This application is required to make an entry of hypothecation or lease or agreement of hire-purchase in RC.

You can download all the forms mentioned earlier online through the Parivahan portal. The following steps will help you regarding the same.

Step 1: Access the official website of the Parivahan portal.

Step 2: From the top menu, choose "Informational Services".

Step 3: Then select "Downloadable Forms" followed by "All Forms" in the drop-down menu.

Step 4: On the subsequent page, you will be able to see and download all the forms associated with the Regional Transport Office (RTO).
