If your PAN card is lost or stolen, immediately file an FIR incident report at your nearest police station. Once you file an FIR, you will receive an incident process and begin your lost PAN card request procedure.
When your PAN card is lost, you can request a duplicate PAN or opt for a reprint of the card. Your PAN Card number and details will not change, you'll only get a new PAN card.
You will need a set of documents in order to apply for your lost PAN card. These documents include:

When you lose your PAN card, don't sweat! Here's a step by step guide to how to apply for a lost PAN card.
Go to your closest police station to raise a First Information Report FIR. This FIR will include details of your PAN card loss, as well as collect your personal information such as your address and phone number.
Go to the Income Tax Department of India online website, https://www.incometaxindia.gov.in/. Navigate to the "Reprint PAN Card" section and select it. You will have to fill out a Form 49 application in order to apply for your duplicate PAN card.

Since a PAN card is proof of your identity and linked to your finances and banks, you will need to provide the necessary documents in order to verify your identity and make sure no fraudulent requests are being made. Usually, the documents you need to submit will include a copy of the FIR incident report, your AADHAR card, and 2 passport-sized photographs.
When you submit your AADHAR, an OTP will be generated and sent to your registered mobile number. Verify the OTP to move the lost PAN card process forward.
You have to submit scanned copies of the documents mentioned above. This is done through the e-sign process. AADHAAR needs to be uploaded as well as your photos, signature, and address proof.
After you upload your documents, you will have to select your PAN card type. You can choose between a physical PAN card or an e-PAN card. For an e-PAN card, you will need to give your email address so that your digitally signed e-PAN can be sent.
Note down the token number and PAN Acknowledgement number you will be given. These numbers will be used throughout the lost PAN card process.
Use PAN Acknowledgement Number
Every PAN card comes with an acknowledgement number when you apply for your duplicate PAN card. You can use this number to track the status of your application, as well as use it as a temporary PAN card for details.

Go ahead and download and print an application for “request for new PAN card or/and changes or Correction in PAN Data”.
You need to fill the form out in only BLOCK letters and BLACK INK, as per the requirements.
Enter your 10 digit PAN number
Attach 2 passport size photographs and cross-signed them, making sure you sign across half of the photo and half of the form, making sure to not write over the face.
Once you're done filling up the application, send it in, along with the payment, as well as proof of identity, proof of address, and proof of PAN.
